Do you tour with your own sound engineer?
This question sets the tone of the whole rider — that's why it comes first.
It's not that you can't ask for a specific mic — it's that it rarely pays off. Think of it the other way round: how would you feel if someone told you which guitar to play? The venue's engineer knows their mics the way you know your gear. Name the type («dynamic for kick»), add your preference if you have one, and let them pick the tool. It sounds better and they'll thank you for it.
The venue needs it to know who to expect and which posts it has to cover. Anything you leave unticked means «does not travel».
Add as many as you need: whoever picks up the phone, the engineer who arrives early, whoever runs the lights.
